One and the same certificate in Hyderabad, Telangana

Hi all. Please help me with this:
My current passport has my name as – “Abc Xyz”,
but my Degree and other educational documents along with my birth certificate have my name as – “A Xyz”. Surname has been shortened to just the initial in all the documents other than Passport and Aadhaar card.

And to apply for a Declaration of Value certificate to study in Italy, one of the requirements is a “One and the same” certificate in case of discrepancies in the full name across the documents. They explicitly mentioned that an affidavit will not be accepted, they need a certificate apostilled by MEA.

If someone could tell me where I can get this
“One and the Same” certificate in Hyderabad, Telangana or if you can think of any alternatives, I would be really grateful.
