ICICI Bank approved a mortgage insurance loan without my consent

I recently applied for LAP with ICICI Bank and yesterday I got to know that a mortgage insurance loan of 1.2 lac has been sanctioned without my consent/approval. I was not even aware of this

I have submitted an complaint letter in my home branch. What actions should I take in case they fail to revert the loan??
