March 7, 2025 at 3:18 pmMy dad died on the 5th of fab. We were so heartbroken that we forgot we had to apply for a death certificate. In order to get his pension or job, a family member said we could take care of that, but now no one has even bothered to make a single call. We don’t have any savings, nor do we have any resources to earn in order to get anything we need a death certificate. Can anyone guide me on how I get it plz ? -

Get atleast 15 original copies of death certificate by paying extra because whenever you go, they ask for original death certificate.
If 21 days are passed, still visit municipality and ask them procedure

March 7, 2025 at 3:31 pmCan you share more details? Where did he pass away? If in a hospital, the hospital will issue a cause of death certificate. The hospital then sends it to the local municipal office and you can collect it from there on a specified date.Since more than 21 days have passed now, go to the local municipal office and explain the situation, they will help you out. If necessary, do grease some palms as in this time of grief, you don’t want to fight other battles.
I’m sure, you aren’t the first one to have faced this, so don’t lose heart, there will be a way. Also, like another commentor said get 20-25 copies of the death certificate (max they can give) as in banks and other places, they ask for original copies to change the name of the nominee in the account, transfer of SIM etc.

Since youโve mentioned he died at home.
Hospital wonโt issue you the death certificate.1. If the area you live is urban,youโll have to go to the nagar nigam office.
2. If itโs a rural area, youโll have to contact the secretary (VDO) of that area. And he/she will issue the certificate.
If itโs the 2nd case, i can guide you further.

March 7, 2025 at 3:32 pmcontact the hospital that he was taken to, they’ll help you.also pro tip: Now no one’s going to really ” be there ” . it’s just you and your very very immediate family like mother or siblings. The rest will slowly evaporate.

March 7, 2025 at 3:55 pmYou must have a pass for the crematorium right? Without it they won’t let you cremate the body. Also you might have the crematorium receipt.Take those things, and your father’s adhaar, pan card and straightaway go to the municipality ward office and ask around.

March 8, 2025 at 1:31 amI guess you from india so get the certificate from hospital where he passed away then apply the death certificate in your muncipality office cos its the legal government certificate that will be applicable everywhere..The hospital one is only used to get that muncipality one.

March 8, 2025 at 2:06 amSorry for your loss, I have experienced the same situation lately, we had a contact in muncipality, & he did most of the work, the form needed two govt employees as witnesses.Don’t forget to take atleast 10 copies of originals for banks, insurance and other purposes. Notify them ASAP. & look for any govt benefits that your family might qualify for, this will help with making things happen, don’t spend any money unless you have to. wait for it.

March 8, 2025 at 9:04 amSubmit Form 4A (Delayed Death Registration) at your nearest Municipal Corporation office or CSC center. For deaths reported after 21 days but within 1 year:1-Requires notarized affidavit explaining delay (โน50 e-stamp paper)
2-Police verification report (if no hospital records)
3-Two witnesses’ sworn statements.

There will not be anyone who will come to help or rescue you, you are alone and train to fight alone all battles which will make you stronger, and you will be proud one day how you were able get through this alone. All this will pass everything will get settled, just embrace everything and learn to handle things independently, make contacts for everything.
