Marriage Certificate – maiden name not mentioned

We registered our marriage multiple years after the ceremony – as a certificate was not required for years. I retained my maiden name, but our agent put my partner’s surname instead of my parents.

Now I am stuck with a document which has my post marriage name – however I still use my maiden name

How to go about solving it?
