Resigned, returned laptop(got receipt) but after 3 months company messed with EPF exit date(need suggestions on how to approach this issue)

I resigned from my last company due to health reasons(documented), the HR people agreed for release during discussion in office. I returned laptop(got receiving receipt) and was asked not to come to office. When after 3 months i asked for my relieving letter they stopped replying. I later reiterated to them that its compulsory to provide the relieving letter which they ignored. Last week when i was checking my epf i noticed they have mentioned wrong exit date for me which clashes with my current employer.I need to get this corrected. What are my options and what should i do now?
