What are the documents I should ask my company after getting laid off?

I got mail from my previous company asking me to sign some kind of form which says I am accepting that I left this company for personal reasons and they are giving my final dues amt xxx.

I dont know my UAN number as it was created in this company, they also didn’t give me experience letter and payslips. I was bothering them so they just gave relieving letter.

And worst of all its been 4 months since they laid me off and now they are giving me half of my salary without telling me where they deducted it.
