Injection for anemia?

Was that iron injection?

If yes then it is very well know to cause allergic reaction/anaphylaxis leading to above said condition. But it is very much manageable condition. I am a doctor and I encounter such reactions a lot but never seen anyone dying because of that. You just need to act immediately.

Moreover these injections are given under supervision and strict monitoring.

Your case looks clear cut case of medical negligence. Fluid in her lungs might be due to acute lung injury due to anaphylaxis.

Almost similar thing happened in my friend circle. 5 meme pages shared a picture of group photo as meme with weird caption. None of the pages responded. They hired a lawyer and sent a legal notice. They asked for monetary compensation as well.
